• To provide healthcare to you

• To provide information relevant to your person/medical condition

• To perform administrative tasks associated with providing healthcare to you such as making appointments and processing accounts

• For quality improvement and service management purposes, however this will not include any identifying information about you.

Hay Health Connect may disclose you information:

•  To your GP, referring health professional or external healthcare services to support your ongoing care and wellbeing

•  To your substitute decision maker or next of kin, if you do not have capacity to provide consent yourself, so they can make health care decisions on your behalf

•  To any agency that may be paying for or contributing to your healthcare

•  Where it is otherwise required or authorised by law

Hay Health Connect may ask to access your information:

• From other healthcare providers, pathology laboratories and diagnostic imaging providers for the purpose of providing healthcare to you unless you withdraw your consent at those agencies for doing so. Hay Health Connect will always seek your consent prior to contacting other healthcare providers.

Hay Health Connect may contact you:

• In relation to services and updates, in regards to the nature of your engagement of Hay Health Connect

• To seek your feedback

• To obtain further personal details as they are required
